I can recognize well all charachters as well…
Some words looks as Polish, yes

 Beat me to it. I see LAT on there, and the word before it looks 

like TRZYSTA and somewhere I looked said that is Polish. 

 Next line may be W JERVZALEM perhaps.

Carogrod, city of kings. what's the city they refer? Why 500 years there… 

Its in reference to a painting of Mary that travelled through different means and dowries the item you hold states its travels. The portrait ended up in Poland.  Somebody has put a full description of the portraits travels on facebook your medal/token depicts the painting and the writing as a brief list of its supposed history/travels. For some reason I can't post a link to the page it gives the full text instead.

Facebook page is for “The National Shrine and Parish of the Divine Mercy Philippines” its under the title of Saint of the Day (Our Lady Of Czestochowa) and the post was listed back on 26th August 2021
